  1. Doctor's Note - If you require a doctor's note, please discuss this with the doctor during your appointment. The provision of a doctor's note is a private service (not OHIP-covered). If the doctor agrees to provide you with a note, payment for this note can be made below. Following payment, your note will be emailed to you. Please note: the doctor will indicate whether a 'simple' note charge applies in your circumstances ($50) or a 'detailed' note charge will apply ($100).
  2. Presciption Refills - If you wish to  email in your prescription refill request to the doctor, this is not an OHIP covered service. Such requests can be processed for you at a charge of $30 per request. Please email the request to [email protected] and please make payment below. When making payment, please make it clear which medication the refill request is for.
  3. Medical Records Request - If you wish to have a full copy of your medical records held by us to be sent to you electronically, an admin charge of $50 will apply. Please email the request to [email protected] and please make payment below.
  4. Travel Medicine - If you wish to see the doctor for a travel medicine or vaccination enquiry, please note that such consultations are not covered by OHIP and are therefore private pay. You can book your travel medicine appointment through our special travel medicine booking link, where payment will be processed online at the time of booking: LINK.
